I understand that you have installed the package "Salesforce Lead Capture" By Salesforce Labs(https://appexchange.salesforce.com/appxListingDetail?listingId=a0N3A00000DrzmfUAB) from app exchange. I tried to repro the scenario in my Org and see the below error in the console
Since it is coming from the managed package, we do not have access to view/ troubleshoot their code. You will need to report this error to the package provider so that they can fix it from their end.
