New Button on Lookup

I would like to create a have a "new" button on the lookup layout for custom objects (just like the lookup for the Accounts object). How would you do this?

for a similar kind of problem, I had written a S-control. You can try writing a VF page and using JS property of Pop-up, call it.

 

I am not sure how the coding will work in VF but it had worked fine in S-control.
