Creating a Question and Answer Bank

Has anyone created a question and answer bank sort of like creating a form.

You can use survey force app from AppExchange which in non-managed app. you can customize its functionality. You can create question and take answers from users.

Thanks Waqar that option did cross my mind but didn't need the added functionality of the rest of the package.

Since the survey force is managed package, you can remove other functionality and can just use the question creation and Question answer vf page functionality.
