I'm getting error
Can't find 'Last Name' and 'Company Name' screen fields in the 'New Lead' flow.
I saw where people had issues with the lightning component screen, but I followed the directions that was posted on force.com to help with that issue. I don't understand where I have went wrong..screenshots below.

This error occurred when the flow tried to create records: REQUIRED_FIELD_MISSING: Required fields are missing: [LastName, Company]. I just received this email if that helps. I am not sure how they are missing.
On the screen the input fileds must be "LastName" and "Company Name" but from your first screen I can see that you have given Company instead of Company Name.
