Managed Package Upload Fails Due to Permission Set

Code coverage in our Developer org is 89%. Uploaded package today and uploads fine. When we add a Custom SystemAdmin Permission set, upload fails with 5% coverage. Any ideas why that might be or how to troubleshoot?

Remove the access to the custom permissions from the permission set, upload a new version, and install the new version.

Unfortunately no because we don't have any Custom Permissions in this permission set.
