B. When Workflow rules are involved, triggers are executed again. So there is a very high chance Workflow rule is a culprit here.

In my opinion, no. Adding the System.debug() wont help to understand the cause of the second execution of trigger.

In my opinion, the answer should be 

  C. Add system.debug() statement to the code and uses the developer console logs to trace the code
  
* Because the trigger is not working for the second time instead that trigger creates two Order while its execution
