What is the CTI vendor name?Did you reach out to see what the Adapter provider have to say about the issue?
If the adapter provider confirm that the issue is not with their adapter, then can you try to replicate scenario in the reported org using Salesforce provided Demo adapter so that we can isolate that this is an issue on salesforce side?
Incase you have a premier success plan with salesforce, have you tried logging a case with salesforce support team for more inputs?
Thanks
