Trying to create a questionnaire

Hello everyone,
I'm a new to salesforce, and couldn't believe the possibility and the support from the salesforce community, I'm glad to join, and looking forward on creating something awsome here. 

I was searching around on how to create different questionnaire (one with yes or no answer, and the other one to score 1-3), and at the end I want to calculate and populate the result and flag it for (red label for example) if the score is greater than certain amount.

Can I make this possible with cutome object.

step1: Design VF page for targerted with that custom object OR use public site that is already developed for  this. 
Step2: create any temp object to store that result info( if you need to track activity info). or make these in to cache and display it. while closing browser remove the cache. 
stpe3:make any condition that Developer VF page will popup to user to complate survey. 

i believe that can solve your problem please let me know need further details on this
