Web-To-Activity???

Hello-
 
I need to create a form (similar to web-to-lead) form that submits a Contact/Lead's activity info into a corresponding SF activity.  Can this be done?

Via the API, yes.  This pattern is called Web2X, and examples of it are available in Java and .NET.

Can you clarify - is there a way to create a new record in an SFDC instance without verifying the user through an API Login (like you can with Web2Lead)?

No, the app server you use to create your Web2X needs to login to the API and have the appropriate credentials (unlike Web2Lead, where  the service provides that for you.)
