Web to Lead help

I need help implementing the web to lead functionality. Does anyone have a sample web to lead form that you have used successfully on a web site. I just need a sample. I will be able to take it from there. I really appreciate it. I am having problems with my web master.

if you're just looking for HTML, you can get this from the interface.  Go to Setup | Customize | Lead | Web To Lead - then choose "generate WebToLead form"...is that what you're looking for?

I know how to generate the code from salesforce. I need a sample form from a web site with the salesforce web to lead code in it so I can get started on the actual web to lead form

Hmmm, I'm afraid I'm not following your question.  The HTML that you can grab from the interface is essentially one web page - it can be dropped into a site and it has all of the information that you need to start collecting leads in sfdc.  It is a working webtolead page.

I'm sorry I am not making myself clear. I have generated the web to lead code from salesforce and need to know where to drop the code in my web form in my web site. Does anyone have a sample they can share?

Here is another example.  http://www.interactiveties.com/contact.php  This one is much simpler that the one jchen built but generates a lead in my Salesforce.com instance nonetheless.
