I think I installed it from the MSI install link in the Office Toolkit Documentation section here though I know it then seemed to revert back to 3.0 when I tried it again on a different machine...so not sure which version is currently posted.
I assume the Outlook Sync now uses it, so try installing that update and see it it updates the toolkit as well...
Failing that, let me know and I'll post the dll somewhere, and you can just copy and register it directly...
I can confirm that installing the current "Connect for Microsoft Outlook" from the Salesforce GUI will also give you Office Toolkit 4.0 (although the Connect for Outlook version itself is 3.3)
