This is done automatically for Custom Objects the first time they are installed. As for Standard Objects, you can package a custom page layout, but after the install, the admin will need to assign the new page layout to the profile of users.

Yes, you could use the Metadata API to automate configure certain customizations. The entire list of components supported via the Metadata API is here.
