Quick Create Window Visibility Issue

When a contact is marked for sync in outlook and the account is not already in SFDC, user must select Quick Create to create the account. However, I can only see the top half of the Quick Create window (Account Name, Site, Phone). Website, Save, and Cancel are not visible. I have changed resolution settings but problem still exists. Any ideas? Thanks.
