need help VF page that allows Customer Portal Users to upload a document to the DOCUMENT object

I am losing my mind trying to find the answer to this question.

 

I want to put a link on my customer portal page that opens a new page that allows a customer to upload a document - and store that document in the DOCUMENT object (document tab), in a specific folder.

 

I can't find anything.  I found one example, but when I entered it, it seems like only partial code.

 

Can anyone help?

Thanks - when I run that code and view the page it tells me I have to log in :  See error message:

 

Authorization Required

You must first log in or register before accessing this page.
If you have forgotten your password, click Forgot Password to reset it.

you are trying to use that code with Sites or customer Portal?

Have you enabled that page in customer portal profile? It will throw an error like authorization required, if your page is not enabled in customer portal profile.

Yes, I am putting this up as a site (link from the customer portal).

 

I'll try enabling for portal first and let you know.
