Mass Update Task Due Dates App Not Working

I installed the Mass Update Task Due Dates App, but when I click on the link the top of the page that is supposed to display the user, dates, etc. is empty. Does anyone know of a workaround.

I use IE 7.  Nothing is displayed between the "Select user and date range for task list" top and bottom lines.  There is no error message pops up, but "Error on Page" is shown on the status bar.  Clicking on it reveals the following:

Error:  Permission Denied

Code:  0

I hope this helps diagnose the issue.  I can supply screen shots as well.
