When using email-to-case, attachments on emails will be captured and included in Salesforce. The attachment will not be located in the Notes and Attachments related list on the Case object. You need to go to the email in the “Emails” related list and open the email to see the attachment:
Scroll down to the “Emails” related list and then click on the link in Subject to open it. Once you click on this you will see the attachment on the email.
I think the attachments are saved in the Open Activities related list. Scroll down to the Open Activity and then click on the link to open it. Above the task detail, the line "This task appears to be associated with an Email Message. Click here to view the associated Email Message." Once you click on this you will see the attachment on the email.
Search Appexchange for apps that copy attachments to your case, like http://appexchange.salesforce.com/reviews?listingId=a0N300000016cOVEAY
Create a custom enail service that replaces email to case and also copies over the attachments.
The email sent from case will automatically add in attachment under case related lists.
