I need a NEW CASE button on the Opportunity Layout that maps to opportunity fields - why is this not an option??

You can probably create a custom look-up field in your Case object, and then configure the related list to appear in your Opportunity page layout.  This related list wouldhave a New Case button on it, depending on how you have your profile configured.
