Error message when typing in lookup fields (in regular SF UI...nothing custom)

Here is the error message: error: {faultcode:'soapenv:Client', faultstring:'Attribute "xmlns" bound to namespace "http://www.w3.org/2000/xmlns/" was already specified for element "query".',}

what browser are you using? last time i saw this was when i was using IE8?

It's occuring in Chrome and IE11. I found that thread related to the issue in IE8...I can't find any other ideas/solutions anywhere! The real annoying factor is it's not consistent. It doesn't happen every time.

if all users are suffering then i'd log it with salesforce, if it's not a VF page then they need to troubleshoot it.

Is it on all objects? or have you noticed it specifically with accounts.

also do you have any apps installed that do auto lookups and checks to see if contacts or accounts exist?
