Salesforce for Outlook (side panel) publisher icon not appearing

Does anyone else have issues with the publisher icon (+ icon) not appearing in Salesforce for Outlook?  I need the ability to create Cases.  I can't seem to troubleshoot this.  

I have all of the system requirements:

I have followed these troubleshooting instructions:

I have upgraded to IE 11.
I have uninstalled and also intalled the 2.5.1 version of Salesforce for Outlook.

The icon is still not showing up !!!  Please help.

I finally got this to work.  My company is stuck on Outlook 2007.  If you are in the same situation as me, dont upgrade to version 2.5.  This downloads MS Office 2010 Primary Interop Assemblies file which i have a feeling is not compatible with Outlook 2007.  If you have upgraded to 2.5 and want to move back to 2.4, use this link (https://help.salesforce.com/HTViewSolution?id=000194302&language=en_US).  You will also need to manually uninstall the "MS Office 2010 Primary Interop Assemblies" file from your program.  

My recommended solution may not work for everyone.  I am just sharing what worked for me to fix my issue.

Hmmm, couple of questions:
What edition are you working on? Group, Professional, Enterprise--that determines where the layout is managed.
Is the publisher action set up and working in the mobile app or in chatter?

We are using Unlimited.  The publisher action is showing on Chatter, and mobile app.  I still dont see the + icon on the side panel.

I'm going through the process of setting this up, I'll let you know what happens. 
Real quick, you have a created a Outlook Publisher Layout under Setup/Outlook Configurations and have assigned it to the user that you are testing?

Hmmm, it seems to be working fine if I have an outlook configuration set up and have an outlook publisher layout assigned to my particular profile. Let me know if those are set up on your end as well and you still have the issue.
