Hi, one of my employees keeps having to reset her password every time she logs out. Any idea why this is happening?

This is the error messege:

Your login attempt has failed. The username or password may be incorrect, or your location or login time may be restricted. Please contact the administrator at your company for help.

The Process: 

Try to log in
The error comes up
We reset the password
Get logged in with new password
Log out
Repeat

That happens if the URL keeps taking the User to the *same* reset password page(over and over again).  It isn't obvious by looking at the URL. Ask the User goes to go to login.salesforce.com in a *different* browser.  That will give them a fresh login URL to SFDC - and login successfully.
If that works, they need to reset their cookies in the original browser so they are not re-using the Reset Password URL.
