Hi Rashmi, You can selecte the select the Content Source of Custom Button as OnClick Javascript and include below code in the formula.

alert('Checkbox is Unchecked');
Here, Test__c is the API name of the checkbox on my Opportunity page. You can replace it with your checkbox name and also alert message.
