Trailhead Error on Process Automation Track "Challenge Not yet complete... here's what's wrong: There was an unexpected error in your org which is preventing this assessment check from completing: System.ListException: List index out of bounds: 0"

Challenge Not yet complete... here's what's wrong: 
There was an unexpected error in your org which is preventing this assessment check from completing: System.ListException: List index out of bounds: 0

Please help.  Error comes up when trying to do the Advanced Admin Trail module > Process Automation > "Routing Records with Approvals".  

I really want to complete this challenge - please help asap.  Thanks!

This happened to me too. It turns out, I had put the entry criteria both in the "Entry Criteria" section for the Approval Process, AND in the criteria for the "Approval Steps" step 1.

Once I removed the criteria from the Approval Step and insted selected "All records should enter this step.", the error went away and my challenge was completed.

I got the error because I forgot to activate the approval process. Once Activated, the error was resolved.
