Scenario

Hi all,

I have a scenario as below.
Doctor needs to select the Medicins to patient by selecting 3 fields(Medicine Name,Quantity,Duration).
I need a Button (Add Medicine) when i click then the fields(all 3) should display
When i click Add Medicine 2 times two rows of Medicin selection should be display.
Please advice me how can i achive this.
